Application status vs. Application process status vs. Patient status

Application statuses are the main statuses for patient application. They are different from application process statuses as these apply to each step of the application process. They are also different from the patient status, as they can apply to one or several applications for the same patient.

Application Status

Draft

The 1st application has been created in the PMS and the patient is going through the steps of the application (consent, medical or financial eligibility, …) The patient is not enrolled in the program yet, but can be considered referred, assessed, eligible, …

Closed

The application was stopped at any point in the process for non-eligibility reasons, or lack of interest in the program.

Active

An active application corresponds to the current application (consent, documents, PFET score when applicable) for the current treatment for the patient

Treatment on hold

Axios has activated the “on hold” feature, putting the application (and treatment) on hold for 28 days. The treatment can restart at any point during those 28 days or be prolonged if the physician suggests continuing the hold.

Application Process Status

Each step of the application has a status:

Recorded

The patient’s information has been added to the PMS.

Referred

The consent (patient and/or physician) has been uploaded and step 1 of the application has been validated.

Introduced

The program introduction has been completed and validated.

Medically eligible

The Medical Eligibility step has been validated. Should the patient not be medically eligible, they would receive the “not medically eligible” process status, and their application status as well as status would be “Closed”

Financially eligible

The Financial Eligibility step has been validated. Should the patient not be financially eligible, they would receive the “not financially eligible” process status, and their application status as well as status would be “Closed”

Interested by the program

The Interest in the program step has been validated. Should the patient not be interested, they would receive the “not interested” process status, and their application status as well as status would be “Closed”

Axios to provide documents needed

Axios needs to provide the patient with the necessary documents to continue the application process.

Pending documents

Axios is awaiting all required documents from the patient

Documents completed

All documents have been received and validated by Axios. The patient can move to the financial interview step when applicable, or directly as Eligible and to the enrolment step, depending on the program design.

Financial interview completed

Axios has ran the financial interview, and the corresponding PFET score has been entered in the PMS.

Patient is informed about financial results

Axios has informed the patient about the results of the PFET results, and their implication in the program.

Patient appeal financial results

The patient has decided to appeal the PFET results. The process will go back to the previous step to allow to enter the appealed score into the PMS.

Eligible

The patient has completed all previous steps of the application and is eligible for the program.

Patient needs charity support

The patient is requiring charity support to support payment during treatment.

Pre-enrolled

The patient has confirmed continuous interest in the program and completed all application steps.

Enrolled

The patient is pre-enrolled, has a start of treatment date, and has acquired the first MedPack of their treatment. This status is triggered when first MedPack is validated.

Dates and Triggers

The Patient Management System records the dates at which each patient changes its status

Date of referral

Date at which Axios has confirmed to receive the full SMN (signed Patient consent, signed Physician consent and the completed application form). Referrals could come from a Physician or a Charity. The date of referral starts the Application process. This application is valid for 1 calendar year from the date of referral, after which the Patient will be considered referred but not enrolled, and a new application will be needed

Date of first contact

Date at which Axios has first contacted the Patient after referral

Date of assessment

Date at which Axios has conducted the Patient assessment and the results have been communicated to the Patient

Date of complete documents received from Patient

Date at which the Patient has submitted all application documents (financial, ID, insurance, etc.)

Date of purchase

Date at which the Patient has purchased the first box of medication in the Program

Date of enrollment

Date at which the Patient is enrolled in the program, and defined by the date of acquisition of the first MedPack

Date of start of treatment

Date at which the Patient has started the Treatment Plan, and defined by the date of administration of the first MedPack. Can be the same as date of enrollment

Date of stop of treatment

Date at which the Patient has confirmed he/she stopped taking treatment

Date of Drop-out

Date at which Axios has confirmed the Patient is out of the Program and will not complete treatment through the Program

Years in the Program

Number of renewals + 1
