How to refer a patient in MyPatients, step by step
Patient Referral Step by Step
Through a simple interface and 5 steps, physicians can also refer patients directly from the MyPatients.
Step 1

The first step consists in selecting the program they recommend to the patient.

Once the program is selected, they can select the appropriate indication for this patient.

Finally, the Physician chooses the relevant prescription.
Once the program, indication and prescription are chosen, the physician then enters the actual dosage and unit for the treatment. Should any of the information be missing, the physician is prompted to complete the screen before moving to step 2.
Step 2

Step 2 is information about the patient for the purpose of patient management.
Information relates to the:

Patient’s name
-
Date of birth
-
Gender
-
Location
-
Referring hospital
-
Contact information
Additional information such as their insurance situation is also captured.
Step 3

Once patient information is entered, step 3 captures the physician’s consent. After reviewing the Statement of Medical Necessity (SMN), the physician provides a series of consent related to signing the SMN, agreeing with the patient to be contacted by Axios, and providing a completed SMN for record.
The physician then electronically dates and signs the referral.
IMPORTANT
In countries where the electronic signature is not recognized, a copy of the signed SMN will be mandatory.
Step 4

Similar to the physician, patients can consult the SMN, and then agree to a series of statements under patient consent.
Patients then electronically date and sign the referral.

Step 5

Lastly, physicians are encouraged to upload a copy of the signed SMN through the application. By providing this copy, they ensure patients get the support they need as soon as documents are validated by Axios. It is Axios’ responsibility to verify and validate all received uploads before reaching out to the patients to introduce the program.

The upload functionality is the same across the applications. Permissions prompts will occur on first upload. All uploaded pictures are received by the program staff and directly in the patient file that was automatically created by the referral.

Physicians can review the photos before submitting, and retake if needed.
Referral Complete

Upon completing the referral, the physician is notified in the application that the referral is successful, and the patient becomes visible in MyPatients. Simultaneously, the patient appears in Axios’ PMS, and a task is assigned to the program team to review the referral.
Additional security features are present. Should the physician exit the referral before completion, they are prompted to confirm before losing all the information already entered.
Data privacy is of the utmost concern for Axios. Because health information is sensitive, physicians are disconnected automatically from the application after a period of inactivity. When this happens, they are notified in the application.
