Program Application Mapping

Referred	Patient referred by Physician to Axios. Axios has received a signed SMN from both the Patient and Physician.
Assessed	Patient means/situation have been assessed by Axios. Results have been communicated to the Patient
Eligible	Patient fits the Program’s eligibility criteria, has provided all necessary documents, and has confirmed interest to be enrolled.
Enrolled	Patient has started the treatment in the Program.
Active	Enrolled and following the course of treatment
Treatment of hold	Patient’s treatment has been suspended by Physician. Can be continued per Physician’s decision, or Patient can be dropped out. This is a temporary status with a maximum length of 1 month
Dropped out	Patient who was enrolled in the Program and has left before the end of treatment
Complete	Patient has completed the course of treatment and has successfully finished their enrollment in the Program
