If the physician refers from their own device, how does the patient consent? 

In step four of the referral process, physicians are encouraged to read out the checkboxes on the screen to the patient. Patients must be given the chance to read the clauses of the SMN (if they choose to). 

Physicians are recommended to allow the patient to sign the consent using the screen.
