Navigate to Patients

Add a new Patient
-
Input all basic information: Country, Area, Name, Gender, Date of Birth, etc.

Check for duplicates, and if the Patient does not already exist, save
What should I do in case of duplicate?

Overwrite existing data with new patient’s data (as patient cannot be deleted and ID has to be kept)

Contact PMS support if application needs to be reopened
Completing a new Patient's contact information

View/Edit your Patient

Under the Contact Information Section of the "Information" Tab:

Add the Patient’s email address (cannot be an @axiosint.com email)

Add a phone number
-
Make sure to select the correct country code
-
Only numbers are allowed for the phone number (no space, hyphen, etc.)

At least one number is mandatory

Select the preferred method of contact (phone, SMS, email)

Enter the Patient’s previous ID from a previous Patient tracker (if applicable)

Update the Patient file
Adding the Patient's caregiver(s)

View/Edit your Patient

In the Information tab, scroll to “Caregiver contact information”

Enter the caregiver’s information, making sure to use the correct country code

You can add as many caregivers as needed. Be conservative and only enter the relevant caregiver.

Update the Patient file
Allocating a treatment to your Patient

View/Edit your Patient
-
In the Information tab, scroll to “Treatment”

Select the hospital where the Patient is or will be treated (referring hospital)

Only Health Centers located in the Program country will be available
-

Select your Program
-
Select the referring Physician
-
Only a Physician with the same specialty as your Program’s combination will allow you to allocate this Patient to your Program

Select the appropriate Indication and Prescription
-
Choose the appropriate dosage of medication for the initial treatment, considering the total daily dose. If the Program involves a combination of two medicines, also include a second dose.

Update the Patient file

Previous Treatment

To fully understand the patient journey, we can keep track of when the patient started on the treatment (“Overall start of treatment date”) in case they started before joining the program.
We can also keep track of how many cycles they have completed outside of Axios and of the Baseline Reported Compliance self-reported by the patient (only if patient was on the same treatment prior to starting with Axios).
Insurance/Financial Situation

Patient's insurance and financial situation situation can be captured in this section. None of these fields are mandatory.
Patient Management

Here is assigned the person responsible for this patient. This is important for program management. In the list are only available users that have access to this program.
Program Services

Services are defined at the program level and subservices can be tagged at the patient level to differentiate between patients (for example: FOC scheme vs Voucher scheme).
